REGISTER WITH THE PRACTICE?
When you register with the practice you will be given a new patient questionnaire. Please give as much detail as possible. This information with help us until we receive your medical records from your previous doctor.
If you are on repeat medication you should make an appointment with the doctor before you need your repeat prescription.
It is your right as a patient to express a preference to receive services from a particular clinician. Any preference must be put in writing. We will endeavour to comply with any reasonable preference.
